1 Diagnosis of rare and irregular cardiac disorders
In cases of patients suffering from rare and irregularly occurring cardiac disorders, in which the classical diagnostic Holter type methods failed, event-holter (EHO-MINI) devices are recommended.
They can be used for a very long period of time, up to effective registration of ECG abnormalities/ incidents.
A doctor rents a patient small mobile device that patient wears all the time. At the time of bad feeling, eg. experiencing pain, palpitations or arrhythmia, the patient presses a button on the device. Research goes to the on-line Telemedical Platform, where it is available to a cardiologist or is directed to the Monitoring Center, where it is described.
This allows for highly efficient detection of very rare and sporadic cardiac disorders. After detecting irregularities the doctor and the patient receive reliable material, on which to apply an effective method of treatment.
Such a model has been running successfully at the Cardinal Stefan Cardinal Wyszynski Institute of Cardiology in Anin.
This model would also work to a GP, who rents a device to a patient with suspected cardiac disorders. Research from a device goes through Telemedical Platform to a cardiologist, who makes description and returns the examination to the GP, so that it is possible to make a preliminary diagnosis of patients with the need for specialist treatment. This has a significant impact on shortening queues and greater availability of medical specialists for those who are truly sick.
2 Long-term telemonitoring
Model directed to patients under the care of a cardiologist.
A doctor offers a remote service like long-term cardiac monitoring in addition to traditional medical care. A patient regularly, but rarely, at the in time and frequency consulted with the doctor before, performs a number of tests – usually several times a month. The study goes directly a the Telemedical Platform, where they are available to the doctor. The doctor can get SMS notifications on new research and watch them on chosen dates. The doctor responds only if the ECG is abnormal for the patient.
This provides better control of the patient between visits and make an opportunity to automatically respond to incoming signals and to intervene quickly in the event of disease progression, eg. modifying the dose of medicaments. It also lets to reduce the amount of unnecessary medical visits, which translates to greater availability of a doctor to other patients.
It is also beneficial for a patient, who plans a visit at the right time, which makes them not exposed to unnecessary journeys or leaving work. This significantly increases the quality of life and gives the patient feeling of being under permanent care of the doctor.
3 ECG preselection
Model addressed to cardiologists for rapid diagnosis of rare and irregular cardiac disorders.
A doctor hires a patient EHO-MINI event-holter for 7, 14 or 30 days. The device performs ECG every three minutes, without the participation of the patient. Recordings are automatically sent to the Telemedical Platform. Then they are viewed and selected live by cardiologists at the Pro-PLUS Monitoring Center. Cases in which irregularities were found, are identified and after the preselection submitted in a report to the doctor or the patient.
This provides more than 80% relative detection of existing disorders due to the large amount of research. At the same time, thanks to the preselection service performed by specialists from the Monitoring Center there is no need to view and analyze a large amount of records. The doctor gets only significant studies, based on which effective treatment is settled. Model significantly enhances the work of specialists.
4 Home care for a patient and telemonitoring service
Solutions offered by Pro-PLUS are also widely used in a treatment and care of elder people in their homes. They are particularly useful for doctors and nurses providing home visits or constant nursing care.
By using EHO12 and EHO-MINI devices and Telemedical Platform a nurse and doctor are able to comprehensive care in patient’s home, to provide more medical services and remote telecare.
The nurse can perform 12-lead ECG in patient's home by EHO12 device. Results are automatically sent to the Monitoring Center, where the cardiologist describes them on a regular basis. This study goes after a few minutes back to the nurse. This way the nurse receives reliable information about the state of health their patient during the same visit and can properly respond to a message of feeling pain in the chest or dyspnea. It eliminate unnecessary call for an ambulance or a need to transport the patient to the hospital.
A research can be also performed periodically prophylactically in treating elder people with cardiac disorders. The research may also go to the cardiologist, who takes care of the patient. Thus, it is possible to better patient management and faster response to progression of the disease, eg. by modifying the dose of medicaments, and to avoid unnecessary medical visits, especially burdensome for the elderly or lying people.
By using of EHO-MINI event-holter device and Telemedical Platform the nurse and the doctor can remotely monitor their patients in terms of cardiac disorders and other parameters such as pressure, weight, saturation, etc.
The nurse sets the frequency of the patient taking researches and their type. A research done by the patient automatically goes to the Telemedical Platform, where it is available for the nurse.
The nurse monitors the recordings from any number of patients. Remote access to the data enables to often observing the results of the patients, responding to signals from them and, if necessary, to send received research to the physician for consultation.
5 Telemonitoring of the elderly
This model is similar to the model described above. In addition to monitoring of selected parameters such as ECG, body weight, saturation and blood pressure device provides continuous communication with a doctor and caregiver to monitor where the patient actually resides and send alarm signals in case of a fall or leaving the target zone.
EHO-MINI device is additionally equipped with the following modules:
• voice – providing the opportunity to talk with a patient directly through the device. EHO-MINI is equipped with a button, under which is programmed emergency number (eg. to a nurse, who cares for the patient)
• geolocation – a caregiver or a nurse will get up to date information about where the actually patient is
• geofencing – setting called "safe zone", in which the patient can move. After crossing this zone, an alarm signal is sent to the nurse or caregiver (for patients with Alzheimer's)
• fall sensor – notification to the nurse or caregiver about the fall of the patient, eg. out of bed
This model provides continuous monitoring of the elderly and provide sending important signals to the caregiver, doctor or family members. It significantly improves the quality of life of the monitored person and immediate family, which constantly react in an emergency.
6 Prevention
Model is dedicated to healthy people at risk of cardiac disorders, working in stressful conditions and amateur athletes (eg. running marathons).
A patient performs a number of tests (with doctor's permission) using equipment purchased from the doctor. Results are sent automatically to the Telemedical Platform, where they are collected and evaluated by the doctor.
As the result, a patient gets a feeling of safety andbetter quality of life. Detection of cardiac abnormalities is possible at early stage.
